A realist process evaluation of an intervention to promote competencies in interprofessional collaboration among interdisciplinary integrated care teams for older people: Study protocol.

Background: International policy is increasingly committed to placing interdisciplinary team-working at the centre of health and social care integration across lifespan. The National Clinical Programme for Older People in Ireland has a critical role design implementation Person’s Service Model, which aims shift delivery away from acute hospitals towards community-based care. Interdisciplinary Community Specialist Teams older persons (CST-OPs) play an important this service model.
